Job duties:
- assisting with interviewing and hiring new staff
- coordinating orientation plans and onboarding of new employees
- managing PTO requests and KRONOS entries and approval
- CDI metrics analysis and review
- making daily CDS assignments and monitoring individual CDS productivity as well as monitoring CDI coverage rates of priority payor mix
- auditing individual CDS review and query processes for compliance and performance improvement for new and existing CDI staff
- providing individualized educational feedback including in-person rounding and chart review shadowing
- assist other CDI leadership as needed
- manage a case load of 50-150 patients including Medicare, Medicare HMO, Medicaid, VA, and commercial insurance pt
- process OASIS assessments to develop and coordinate patient plan of care with interdisciplinary team
- direct, supervise, and coordinate, patient care team consisting of RN, LPN, and aides
- assess patient eligibility for recertification and schedule home visits base don patent needs and physician orders
- monitor nursing documentation via chart audits and educate staff as needed
- provide counseling and disciplinary as needed
- perform home visits as needed
