Community/Home Health Assertive Community Treatment Registered Nurse

• Proficient in ability to evaluate clients upon admission to Assertive Community Treatment; obtain complete developmental, psychiatric and medical health history
• Monitor vital signs, medication administration and track behaviors
• Meal preparation that meets dietary needs
• Assistance with community integration and travel (i.e. market, doctor's appointments, and other appointments/needs)

• Assess, intervene and stabilize a variety of medical conditions and illnesses with decisive intervention and evaluation of care
• Leadership and preceptor experience through training and teaching provided to new graduate nurses, student nurses and auxiliary staff
• Provide patient education, including educating patients’ family members/significant others about how to manage their illness/injury

• Conduct ongoing individualized assessments as determined by the patient’s condition and/or the client facility’s policies, procedures or protocols and reprioritize care accordingly
• Develop plans of care that is individualized for the patient reflecting collaboration with other members of the healthcare team
• Deliver appropriate treatments as ordered by physician in an accurate and timely manner
