I work with IDD clients that are dual diagnosed with intellectual disability and mental illness. Some of my clients come from addiction and trauma related backgrounds. I have 11 Assisted family living homes throughout the mountains and piedmont. I deal with crises situations almost weekly which consist of many different behaviors and situations. I complete yearly Person Centered Plans ( PCPs). Each individual has different needs and my job is to find the appropriate resources throughout the community to target their individual needs. I help place clients in day programs, jobs, educational settings , support groups and much more. I sign off on monthly billing shift notes , and grids. I must maintain accurate records.

• Monitored compliance with court orders and rehabilitation plans for offenders.
• Conducted risk assessments to determine supervision levels for parolees.
• Developed case management strategies to support offender reintegration into the community.
• Facilitated communication between offenders, law enforcement, and community resources.
• Dealt with Crisis situations on almost a daily basis
• worked in high poverty high crime communities
