Coordinate medical treatment and services Medicaid/Medicare Waiver Primary Physicians and Community resources. Educate member and their families on the MyCare Ohio Waiver services and link them to the needed services and equipment. Perform assessments not he needs of member and evaluate the level of care that each member in to receive.

• Serviced the under 60 population as the Medical Coordinator and Waiver Coordinator for complex cases.
