• Primarily responsible for care management and the complete care of patients with critical care and complex needs using the OASIS system including extensive wound care, trach care/replacement, ventilator care, percussion vest, deep/oral suctioning, GT tube/replacement and tube feedings, colostomy care, SP catheter care/replacement, ADLs, transfers, medication administration, venipuncture, transportation, patient advocacy, care management including determining medical necessity and other duties.

• Provide education to potential patients about home therapy dialysis options in order to prevent rehospitalization, complete home evaluations, determine medical necessity, develop care plan, train, educate and coach patients in performing manual and cycler based peritoneal dialysis in the home using best-practices and established algorithms, perform frequent physical assessments, respond promptly to emergency situations including appropriate interventions, lab draws and lab monitoring, infusions, injections and procedures, evaluate and treat conditions in collaboration with the physician and other team members and facilitate patient care through care management, after hours on-call coverage and telephonic care
• Educate patients about kidney transplant options, assist patient in preparing for transplant application including patient responsibilities, support patient during the transplant process, provide health coaching and advocate as necessary.

• Total care of dialysis patients including care coordination including transportation, on/off, crisis intervention, pre- and post-treatment assessments, medication administration, infusions, care plan initiation and monitoring, patient advocacy, meeting quality measures/peer review, working independently as the nurse in charge of other licensed and unlicensed staff.
