  • 2016 I was hired as case management to roll out Star Kids , as the program developed they changed our titles to SCll for different reasons and continue roll out for several months and again as the program developed I was offered the position one of the first Discharge planner as SK developed, Centene Corporation decided to remove all LVN except the discharge planners , fortunate for me I was 1 of those DC planners, then at that time- several different managers and program changes they put all discharge planners together and made one team for Star Plus and Srar Kids TOC , Sandra Zamora has been a great leader to all extent , current supervisor / manager challenges on every level to excel and prompts us to only be the best we can be. At this time I work with chart/ documenting in True care , Assessments, updating members profile when needed, researching clinicals for DX and further things that may be addressed and assist our members for easier transition from hospital to home , contacting hospital CM and PCP for discharge needs and orders , use Avaya system , team player, daily tracking logs , meet TAT times audit score on average is 100% -see Below for 2023 Centene University Courses I’m Driven, motivated to Excel as Leadership Roles and a changing Lives and possible changing Roles, applying for many different positions for clarity of Good fit / positions available and reaching for Success Fulfilling a Team Role.
