scrub clinic and huddle. Clinic nurse vitals and clinical reminders documentation. Ran clinic. Cover for RN when RN not available. Primarily follow up phone calls. Created my own nurse visits with scheduling.

I was then moved to a FLOAT LPN. Worked all PACT teams if need for coverage. Also phlebotomy/lab and EKG's. TELEHEALTH providers as needed, covered specialty clinics, podiatry, eye retinal screening. MOVE groups as well.

Assigned to PACT Team Primary Care. Clinic Nurse. Hands on care of patient for screening prior to being seen by MD. Vital signs and fullfilling all 'clinical reminders' in CPRS. Provide immunizations and EKG's as ordered. Participated in WHOLE HEALTH EDUCATION and support groups. Collateral duty : Adaptive Sports Outreach and support. Participated in group 'Shared Medical Appointments' (SMA's) for DM and CHF. Covered RN with return phone calls.

During my time here worked Med Surg floors, ER ( 4 years) and PACT.

Unit floor nurse providing acute care to veterans. IV inserts, GT's , IV meds, blood transfusions, suctions, trachs, blood draws.

Accepted 'role out' position in Primary Care with new model 'High Performance Development Teams' PACT. ( see prior description of PACT roles )
