My responsibilities included assessment, coordination, and managing Residents care needs to age in place within the Assisted Living and Memory Care Unit. Creating Service Plans that are individualized and specific to each resident, also including residents’ families to create a whole picture of the resident and their lives.

Delegate staff to ensure the best practice of resident care, from ADLs to medication Administration, per Oregon Assisted Living and OBON Regulations. Documented and taught documentation to staff to provide an accurate description of any changes in a resident through to resolution or change of condition involving a new assessment and care plan.

I had continued contact with residents, their families, outside services, and physicians for continuity of care.

I was on-call for all medical and urgent needs and would problem solve these issues with staff, resident, and families and have physician or Emergency involvement as needed.

I worked with residents, families, staff, and outside services to provide end of life care that would allow residents to stay in their homes at their wishes.

Facilitate day to day operations of a small coastal dialysis clinic. Maintain the building and regulations there in.

Maintain patient care in accordance to CMS regulations. Assist patients with treatments, education, obtaining resources through SW and RDS.

Monitor staff and ensure all training is up to date.

• I care for a variety of patients from medical and surgical to Hospice, end of life care. Duties include, patient assessments, giving medications, communicating with staff and doctors for the well being of the patient. Communication and documentation are an important part of my job to not only ensure the protection of the employer and myself but to aid to patients and families in the next steps of their medical care. End of life care involves a much deeper family oriented communication and personal time to assist everyone involved in the dying process and care of the patient after they pass.

• My job consists of working at the Carter Juvenile Detention Center with youth that present with multiple behavioral problems. I work with the full staff on maintain medications and administration of medications as well as the healthcare of the youth seeing them when conditions present. I maintain communication with the full time staff via written reports, documentation on MARs, charts, and with the doctors. Contacting doctors for verbal orders and maintaining a working relationship with the doctors that are on call. I communicate throughout my shift with the staff who work direct contact with juveniles and take care of any needs that medically needed.

• Responsible for the daily nursing needs and emergency care in a special education classroom. Duties includes watching for s/s of an emergency due to various medical conditions and administer emergency medications while working with an administrative team to alert 911 and family. Daily medication administration, g-tube feeding, and ADLS. Working with OT, PT, and ST to give care on a daily basis. Daily communication with parents and teacher of the classroom.

Responsible for medication administration and maintaining doctors orders on the Assisted Living Wing. Supervising any GNAs and Med Techs. Caring for any of the outside communities needs with the Life Line System and documenting all activity.

When on the skilled floor responsibilities include medication administration, documentation, and working with the team to care for all of the residents. Informing doctors of any changes or needs of the residents.

• Responsible for triaging patient and family phone calls. Making home visits as necessary to assist in any ongoing issues. Provide palliative care for the end of life patient, adjusting medications following doctors orders, and support for all. Admit patients to hospice services after business hours. Full pt assessments, managing care with doctor, hospice staff, and family. Documentation of all contacts on computer.
• As well as making day time home visits assessing patients and family needs. Communicating and documenting with team about days care.

Responsible for Doctor directed patient care within the Doctor's office ensuring they receive needed chemotherapy and infusions. Tasks include:

Scheduling and administering treatments such as chemotherapy, infusions, injections

Monitoring office drug supplies

Preparing chemotherapy treatment by combining prescribed drugs

Communicating continually with Doctor and staff

Creating and maintaining complete and accurate patient charts

Providing palliative and end-of-life care for patients and information to family members

Responsible for hip and knee replacement post operation patient care. Tasks included:

Assisting patients with Activities of Daily Life (ADL)

Administering medications including Patient Control Anesthesia (PCA) and Epidural (EPCA)

Coordinating with and assisting Physical and Occupational therapists

Creating and maintaining patient charts using unit procedures

Responsible for the operation of the medical/oncology unit's Outpatient Infusion Clinic. Tasks included:

Scheduling patients through physicians offices

Obtaining physicians' orders and maintaining current copies

Creating and maintaining patient charts

Providing patient care services by administering IV infusions and/or hydration, medications (Fe, IVIG, Remicade, etc.), chemotherapy, blood and blood products, injections (procrit, neupogen, sandostatin, etc.)

Administering and teaching about anticoagulant therapy such as TPA.

Monitoring recovery of cardiac catheterization and biopsy patients

Assist with bone marrow biopsy and thoracentesis

Administering cortisone hormone stress tests

Instructing the use and care of central line access

Preparing unit documentation

Responsible for the effective operation of the unit as well as RN duties described below:

Ensuring appropriate unit staffing by having the correct number of nurses and technicians based on the number of patients

Assigning patients to staff members

Communicating and coordinating with the hospital Nursing supervisor about staffing

Transferring patients on and off unit as necessary including assessing the need for critical care

Ensuring proper patient placement within the unit

Communicating with patients and patients' families

Preparing hospital documentation

Responsible for patient care:

Administering IV and IVP medications, chemotherapy, blood and blood products

Managing central line access care and use

Providing palliative care using PCAs

Obtaining written consent from patients for procedures and infusions

Communicating with and obtaining written orders from the patient's doctor(s)

Preparing unit documentation and patient charts

Delegating and monitoring patient care tasks assigned to LPNs and Technicians

Responsible for patient care tasks as assigned by RN. Tasks included:

Assisting patients with ADLs

Providing patient oral medications

Administering simple IVF

Providing tracheotomy care, ostomy care, dressing changes, end-of-life care and support, and post mortem care

Coordinating with RN

Completing unit documentation
