• Prescribe bioidentical hormone replacement therapy for both males and females. Comfortable with prescribing Testosterone, Clomid, Thyroid, Progesterone, and Estradiol.
• Order and interpret lab results for hormone replacement therapy.
• Diagnose and treat common side effects that occur with HRT.
• Treat and manage weight loss patient via asynchronous visits for GLP-1's.

• Provide telephonic/televideo visit for acute care needs. Dispatch to in-market on-call

providers for issues that cannot be resolved telephonically.

• Assess, diagnose, prescribe and treat patients, consistent with standards of care to

avoid unnecessary transfer/acute admissions.

• Communicate and collaborate with members of Landmark’s interdisciplinary team,

patient’s primary care provider or other specialists to deliver integrated care.

• Participate in a rotated on-call program to provide home-based support after-

hours.

• Working with a primarily medicare based population to provide patient care on a

longitudinal basis, delivering proactive chronic disease care in the home setting to reduce unnecessary hospitalizations.

• Assess, diagnose, prescribe and treat patients, consistent with standards of care,

in the patient’s home or via telehealth.

• Participate in regular management/interdisciplinary meetings to coordinate and

collaborate on patient care needs.

• Partner with the patient’s primary care provider and other specialists to deliver

integrated care.

• Participate in a rotated on-call program to provide home-based support after-

hours.

• Obtain a review of medical history, perform a physical exam and diagnose

medical conditions to patients in the home setting.

• Provides patient education and completes care management referral forms to

improve access to care and patient outcomes.

• Identifies unmet needs and gaps in care, interprets diagnostic tests, and

initiates appropriate referral to health plans and outreach to Primary Care

Providers.
