EFS history notes; notes and information in the portal. Redetermination for internal processes compliance. Redetermination done within timeframe and in compliance with rules, policies and procedures.
*Childcare application and authorization forms (referrals): Document a valid childcare referral and authorized period of services for families receiving child protection services, families participating in the Welfare Transition Program (WTP), an families receiving services from a designated homelessness program or a certified domestic violence program, It may also apply to children in the Relative Caregiver (RCG) Program if there is a childcare referral.
*Provide determination of VPK application including; document that each child is 4 years of age on or before Sept. 1 of the program year.
*Timely delivery of services for priority groups (referrals): Demonstrate priority groups, families receiving child protection services and families participating in WTP, a designated homelessness program or a certified domestic program , action on request for services within ten (10) calendar days.
* Provided conditional release supervision for youth(s) release from a commitment program through te Department of Juvenile Justice.
*Senior supervisor over four Re-Entry Counselors.
*Conduct file review every 90 days
