Nursing Assistant

• Using principles of Patient & Family Centered Care develops and maintains a positive working relationship with patient and family, team members, department, and medical center personnel
• Assists patients with activities of daily living, including hygiene, grooming, and bathing
• Respond to patients' call lights in a timely manner, identify patient needs, and provide patients with appropriate responses or communicate needs to appropriate team member
• Performs intentional rounding in collaboration with the RN
• Takes and records patient’s vital signs as prescribed by nursing protocol or physician order
• Measures, and records patient intake and output according to established protocol and order
• Conducts point-of-care tests as assigned and/or delegated
• Communicates any changes or pertinent information related to the patient’s physical appearance and attitude, I & O, vital signs, and point of care testing to the nurse. Records in which the nurse was notified of all abnormal results

Mental Health Worker

• Provides patient care to inpatients or patients in the psychiatric emergency department of HCMC under the supervision of nursing staff; therapeutic 1:1, evaluates, intervenes, and reports to and works with nursing staff to follow individualized patient care plans
• Participates in planning and implementing treatment team recommendations concerning individual and group patient programming
• Follows the problem-oriented method for patient charting or reporting on behaviors observed
• Promotes and maintains a healthy and safe treatment setting
• Provides patient care with ADL’s
• Records actions in the EHR
• Provides milieu support to patients and nursing staff. May provide patient care activities and clerical activities
