Floor nurse, Nurse case Manager and customer service care to soldiers in the US Army Health Clinic, Schofield Barracks' Traumatic Brain Injury Concussion Clinic as well as the referrals from the Soldier Recovery Unit (SRU) and other clinics. Performs duties relating to the nursing care of routine, complicated and emergency cases in the TBI clinic. Screens soldiers and other beneficiaries, assigns triage categories in accordance with urgency of medical complaint in order that are more urgent. Screens Medical Records for past history and inputs data into Medical automated system. Observes patients' physical and emotional status, documenting changes in conditions. Participates in updating patient care plans throughout the tour consistent with recognized nursing standards and in accordance with appropriate nursing care and prescribed medical treatment. Enters information about the condition, assessment of patient's condition and plan of action to carry out nursing care in appropriate medical data systems. Assists TBI Nurse Case Manager with data entry on TBI data base (TBI portal and WEB APP), actively contacts patients or provides list to clinics and educate patients on consults, Lab and Radiology orders, Medications and follow ups. Administer pre and post deployment TBI screening Questionnaires. Works collaboratively with SRU, TBI Providers and Nurse Case Managers to ensure soldiers receive appropriate consults and Access to Care Standards is adhered. Monitor patients and observes, identifies, reports and records changes in objective signs and subjective symptoms. Provides written and verbal reports regarding patients care for inclusion in the patient's medical records; measures, and records vital signs, such as height, weight, temperatures etc. Orders an maintains adequate supplies in patient care area, labels specimen, completes laboratory requests, obtain patient records, retrieves radiology and lab reports, monitor stockage level and expiration dates of available medications and medical supplies. Screens patients with concussion assessment tools to include MACE2, VOMS and Brain Scope. Report to PCM on critical result and record/upload assessment result into patient's chart. Coordinated individualized discharge plans to manage safe transition back into community and home environments. Updated treatment plans on monthly basis with latest intervention strategies and progress notes. Coordinated program referrals for community-based resources. Documented case notes daily and coordinated follow-up for seamless case management. Worked with medical teams, patients and families to implement effective treatment plans. Provided ongoing counseling to help patients deal with conditions and processes. Carried out day-day-day duties accurately and efficiently.
