School Social Worker with 3+ years of experience in Developing and implementing professional development training for educational staff and parents, as requested, maintaining a cooperative working relationship with administrators and other staff members, and Developing Individual Educational Plans for 25 classified Learning Disabled students.
In my role, I address students' social well-being and psychological challenges by providing information and counseling, assisting students and parents in problem-solving, making referrals to other resources, and helping students cope with the changes brought by the pandemic. My responsibilities include:
I offer outpatient behavioral health and substance abuse services for children, families, and young adults, focusing on their psychological, behavioral, developmental, and social well-being.

· Develop policies, and procedures, to assure compliance with state and federal regulation
· Continually prioritize job responsibilities and adjust to paperwork and demands to facilitate an effective Social Services Program.
· Maintains contact with staff from all departments including the Director of Admission, Director, and Assistant Director of Nursing to remedy the diverse problems of the residents.
· Maintain confidentiality of all resident care information according to facility policy and HIPPA guidelines.
· Collaborate with an interdisciplinary team to develop, implement, & evaluate treatment plans.
· Monitor, evaluate, and record client progress concerning treatment goal
· Assist families to understand and cope with the resident’s diagnosis and change in condition and status
· Conduct initial admission & psychosocial assessment for sub-acute & long-term 448-bed skilled nursing facilities.
