- Perform thorough initial assessments and continuous monitoring of unstable patients, utilizing advanced tools such as NIHSS and MEND for stroke evaluation, and managing both hemorrhagic and ischemic stroke complications (e.g., dysphagia, aphasia, mobility issues).
- Monitor post-op cardiac patients with hemodynamic instability; manage and titrate critical drips including antihypertensives, antiarrhythmics, inotropes, vasopressors (e.g., norepinephrine), and insulin per protocol.
- Maintain airways and provide advanced respiratory support using Optiflow, BiPAP, and CPAP for patients in acute respiratory distress.
- Assist with non-invasive ventilation and oxygen delivery systems for patients with conditions such as COPD, CHF exacerbations, and post-extubation respiratory fatigue.
- Recognize and intervene in cases of clinical deterioration such as sepsis, DKA, STEMIs, Afib RVR, and hypoglycemia.
- Initiate emergency response protocols (Rapid Response, Code Blue) and assist with urgent procedures including intubation, chest tube placement, and central line insertion.
- Implement CIWA and COWS protocols to manage and monitor patients undergoing acute withdrawal from alcohol or opioids, ensuring safe detox and supportive care.
- Manage and maintain invasive devices including IV lines, central lines, arterial lines, CVP lines, Foley catheters, PEG tubes, chest tubes, wound VACs, drains, and nasogastric tubes.
- Perform advanced wound care and monitor surgical sites for signs of infection or complication.
- Collaborate with interdisciplinary teams (physicians, respiratory therapists, case managers) to develop individualized and holistic care plans.
- Educate patients and caregivers on current treatment plans, medication reconciliation, disease management, and post-discharge care instructions to ensure smooth transitions to home, rehab, or step-down units.
- Deliver specialized care to patients requiring more attention than med-surg but not as intensive as ICU, effectively bridging the gap between critical care and general care.
- Monitor cardiac rhythms, neurological status, and vital signs to identify early signs of clinical deterioration.
- Accurately document assessments, interventions, medication administration, and patient responses in EMR systems in compliance with hospital policies and regulatory standards.
