Integrated Care Team Pilot Program VMC

  • Since August of 2022 I have been working in a pilot program for the county of Santa Clara Health and Hospital system called the Integrated Care Team. My role as an LVN has been very unique in the sense that we have been working with a special demographic of patients with Executive Functioning Disorders. Our mission has been to work with this population and figure out a better way the Health and Hospital system can better serve this population. I have also taken on an additional role in helping the Health and Hospital system working with custody patients and aiding in their pre-release discharge planning and care coordination plans. My role as a nurse has expanded beyond just clinical care but has also grown into managing and care coordination as well as piloting new programs.
