  • Collaborating with case managers helping to discharge patients that needed additional cares once they were discharged from the hospital. I had a strong working relationship with the physicians at Gulf Coast hospital as well as area SNF’s, home health agencies, Hospice, etc.
  • Scheduling appts for patients that did not have a PCP and patients that had wounds, needed dialysis, IV infusion and patients that had COPD and/or CHF. I scheduled Cardiology and Pulmonology appts for pts within 3 days of discharge to help minimize readmission to the hospital.
  • Attending cardiology rounds on the CPCU floor and worked closely with the nurse in charge of our cardiology program and cross functional team.
  • Helping patients at d/c if they needed DME’s, prescription assistance and housing assistance.
  • Helping with Baker Act and/or homeless patients to get facilities to accept them.
