Primarily serve as an advocate to support patients' treatment decisions and care needs.
Manage daily clinic operations for two fulltime oncologists who see 28 to 37 patients daily.
Triage patient and family concerns and identify solutions for based on clinical experience and standards of care.
Coordinate patient care with both internal and external healthcare providers.
Leverage past experience as a hospice nurse to compassionately assist patients' transition to end-of-life care.
Partner with patients to provide support throughout their cancer treatment journey.
Meet with 2 to 10 new oncology patients weekly to provide treatment plan education, introduce available services and connect them with resources.
Collaborate with institute team members - pharmacists, social workers, dietitians - to identify and overcome barriers to care.
Facilitate discussions regarding advanced care planning.
Train and mentor incoming nurse navigator staff.
Provide symptom and medication management, psycho-emotional support and patient/family education for a 12-15 patient caseload.
Collaborate with multidisciplinary team to provide holistic end-of-life care.
Independently apply critical thinking skills to solve medical challenges unique to home-based hospice care.
Manage 6 to 8 patients during evening shift including admission and discharge process.
Serve as Charge RN 80% of shifts to include attending bed meetings, staffing and supporting staff RNs.
Precepted and trained nursing students and newly hired nurses.
