Works in tandem with a Peer Engagement Specialist as part of the first contact team supporting the 590 Crisis System of Care.
Applies De‐escalation and Crisis Intervention techniques to engage participants experiencing a Mental Health Crisis.
Conducts initial crisis assessment and evaluation using the IM‐CAT (Illinois Medicaid Crisis Assessment Tool).
Participates in the development of Initial Crisis Safety Plan in collaboration with the participant.
Provides direct services, including social and emotional support as well as guiding participants in the use of coping skills.
Follows all safety and security protocols and procedures during the crisis intervention,including any medical and/or psychiatric emergencies.
Works in the field as part of a Mobile Crisis Response Team, providing direct services in the participants natural environment or in other facilities when required.
Participates in ongoing Crisis Intervention, Crisis Stabilization, De‐escalation skills and techniques, and Trauma Informed Care trainings.
Completes all required documentation in a timely manner consistent with agency guidelines.
Bills direct services under the Medicaid Services and Reimbursement Guide when appropriate.
Maintains program compliance with CARF standards, Medicaid Rule 132, Value Options procedures, and OIG RULE 50.
Participates in regularly scheduled program case staffing meetings.
Participates in appropriate agency community relation activities and committees, as
assigned.
Reports to supervisor any participant critical incidents, clinical issues, vehicle and/or building
problems.
Completes statistical forms (1006) on an as needed basis.
Documents accurately, neatly, and consistently every contact with or on behalf of a participant in the participant’s designated chart, as required by DMHDD or Medicaid.
Works in conjunction with agency staff to assure that appropriate referrals are made, follow up services are provided, as well as, emergency psychiatric services.
Participates in internal quality improvement program and external committees as assigned.
Receives clinical Supervision from 590 crisis System of Care supervisor Weekly or as needed.
Assist in identifying resources that will support a coordinated continuum of crisis care for anyone living in the identified coverage area
