Maintain the ongoing scheduling of patient visits for all assigned field staff; communicate updates in regards to revisions and new admissions to ensure continuity and coordination of patient care.

Schedule and confirm all visits based on patient needs, staff experience and geographical locations.

Create weekly calendars of skilled nurses,medical social workers and physical therapist.

Answer inbound calls,providing great customer service in each call, answer emails and team messages in a timely matter.

Work closely with supervisors to ensure staff coverage for absences and holidays.

Maintain the supply room stocked and organize on a daily basis.

Maintain equipment logs updated.

Other clerical duties.

• Assisted patients with daily living activities, promoting independence and dignity.
• Checked patient vitals such as temperature, blood pressure, and blood sugar levels.
• Promoted good oral and personal hygiene by aiding patients with shaving, bathing, and teeth brushing.
• Answered call lights and supported patient comfort and safety by adjusting bed rails and equipment.
• Helped patients with self-feeding and assisted feeding, based on individual needs.
