COMMUNICATION

As a Hospice Case Manager, I must effectively communicate with our Medical Director, Hospitalist, and Interdisciplinary Team to make sure our patients and families have the most holistic experience ever. I must effectively explain disease process and the death and dying symptoms to patients and their families.
