  • We do not have a sample form that we can send, but I’ve included information below your doctors will want to include in their letters. There is no specific format they have to use. The “at least as likely as not” language I have in bold below is the language they should use in stating their nexus opinion.
  • The doctor should state the claim they are providing the nexus opinion for.
  • The doctor should state if they’ve completed an exam for your condition and state any records of yours they reviewed in forming their opinion (STRs, military personnel records, other medical treatment records, etc.).
  • The doctor should state their opinion of whether your condition is at least as likely as not linked to your military service.
  • They need to provide the reasoning for their opinion – This can include the records of yours they reviewed, statements you provided about your condition, relevant medical literature, etc. that they used to form their opinion.
  • The doctor should provide their credentials and contact information.
  • The doctor should sign the nexus letter.

  • I know you are not in the office right now.
  • But none of my doctors are familiar with the manual nexus letter. They stated they know about the fill able ones. I told them the fill in the blank ones are considered weak. They ask if I could get you to send a rough draft or an example of a VA satisfactory nexus letter, so it can be used to see exactly what the VA is asking for and a guide line. It can be a x- client’s letter, just delete the name, SS#, or any information, that is important, all is needed is the letter. When you return to the office, please let me know.

After review, it does look like the signature did not come through maybe due to how it was digitally signed. I will send it to you through our software called vinesign via email.
