Assess patient health status based upon presented symptoms and direct patient to appropriate level of care or acceptable alternative venue.
Reinforce health education, counseling and advice to patients and families and assess patient educational needs, prioritize patient learning needs, identify barriers to learning and use educational tools to encourage non-adherent patients.
Complete paperwork and prior authorizations and schedule appointments for patients.
Track, review and monitor diagnostic test results and assist in management of patient care between visits.

Clearly and thoroughly explain diagnoses, treatment options and procedures to patients and loved ones, checking in to verify understanding.
Provide patient education on health care issues and prescribed medications.
Review patient history to verify conditions and current medications.
Take samples to complete diagnostic and routine assessment tests.
Work with multidisciplinary team to carry out successful treatment plans for diverse acute and chronic conditions.
Monitor patient health conditions to report changes to assigned physician.
Evaluate patient histories, complaints, and current symptoms.
Administer medications and injections to patients.
Coordinate patient transfers to other facilities.
Deliver emotional support to patients and families during challenging times.
Provide diagnosis information, treatment possibilities, disease management tips and wound care advice to patients, loved ones and caregivers.
Respond to patient requests with appropriate clinical and personal assistance.
Support diagnoses by taking blood samples from patients for laboratory testing.
Access Mediports and collect blood samples on Oncology patients.
Start IVs on patients who receive treatments in our clinic.
Assist in getting medical records from other facilities on patients and coordinating care
