Assess the patient's physical and mental wellness, needs, records patient's progress, schedule home visits, follow up with discharged patients, providing skilled nursing services and continue education on diagnosis that the patient may have and coordinating patient care and ensuring optimal outcomes. Working as a case manager includes

evaluating the needs of patients and their families, creating a plan of care based on the patient's needs and following up with the patient for feedback, and I also suggest community services that may be helpful and would benefit the patient.

• Full Time LPN
• Prepare and stock patient rooms, take and record vitals, blood sugar checks, HgbA1c's, PT/INR, give injections, perform EKG's, run urine specimen, draw blood in the lab, get prior authorization for tests/procedures, patient referrals, patient education, pass medications and wound care.

• Part time RN Supervisor
• Overseeing a healthcare team and patient care for 60 patients. Overseeing patient care including assessing, processing, and discharging patients. I also coordinated, evaluated, and managed efforts in planning and implementation of program activities .
• Care plans, Resident Admissions, Wound Care, IV Therapy, Supervising LPN's and CNA's, and Medication Administration.

• Emergency Room RN, Providing care for up to 5 patients.
• Prepared to take care of foreseeable emergencies, prioritizing the urgency of care, and rapidly and effectively carrying out resuscitative measures and other treatment. Performed critical tasks such as triaging patients to determine urgency, taking vital signs, assisting physicians and surgeons, and administering treatment and medication.
