o Assigned as Charge Nurse after 8 months and tasked to supervise 1
RN and 6 LVN’s/Patient Care Technicians and acted as a resource
person for staff members within scope of practice.
o Directs and coordinates work shifts for the patient care staff of all
aspects of the daily provision of safe and effective delivery of
hemodialysis therapy and delegates appropriate tasks to direct patient
care staff.
o Held regular meetings to hear concerns, address issues, and notify staff
of daily updates.
o Review physician orders, collects patient data pre, during, and post
hemodialysis treatment for patients; documents in the patient medical
record and adjusts or modifies treatment plan and notifies physician as
needed.
o Develop interventions, treat adverse reactions to treatment, monitor
dialysis systems, and advise providers of significant changes in patient
condition.
o Administers medications as prescribed or in accordance with approved
algorithms and documents appropriate medical justification and
effectiveness.
o Ensures correct laboratory collection processing and shipping
procedures are performed and reschedules missed or insufficient
laboratory collections.o Assisted in maintaining inventory of medications, ordering medications
for clinic, and calling pharmacy for refills/ordered medications for
patients.
o Discharge patients when vital signs are stable and provide patient
education applicable to individuals medical conditions.
o Assesses, collaborates, and documents patient/family’s basic learning
needs to provide initial and ongoing education to patients and family.
o Documents and updates plan of care through collaboration with the
Interdisciplinary Team.
o Ensures patient awareness related to transplant and treatment modalit
