Duties:

• Provide nursing care to elderly residents including medication administration, wound care, and symptom management.
• Coordinate daily activities for residents, such as social events and recreational programs.
• Collaborate with physicians and other healthcare professionals to develop interdisciplinary treatment plans for residents.
• Monitor vital signs including blood pressure, temperature, pulse rate, respiration rate, weight and height.
• Maintain accurate documentation of patient's progress in the electronic medical record system.
• Educate families on medical conditions of their loved ones and provided emotional support when needed.
• Conduct regular rounds to ensure that residents are receiving appropriate care.
• Respond quickly to any changes in a resident's condition or behavior by initiating appropriate interventions or notifying the physician and healthcare team member immediately.
• Monitor diet, physical activity, behaviors and other patient factors to assess conditions and adjust treatment plans.
