
Work with assigned caseload of 15 clients to provide services.
• Interact appropriately with residents in field and act as a liaison/advocate when needed.
• Implement individual service plans to assist residents in attaining rehabilitation goals.
• Maintain, develop and document accurate Case records, regular progress notes and service plans.
• Conduct daily physical wellness rounds of building.
• AMAP Monitor Medication intake of clients as well as record keeping of each individual daily intake.
• Conduct assessments of residents, including but not limited Functional Assessments, Employment,
Nicotine, Substance abuse etc.
• Provide counseling to clients/residents as required.
• Lead recreational, rehabilitation and counseling groups.
• Escort client in the community to Medical, HRA, and other pertinent appointments.
• Manage crises and intervene appropriately to minimize risk of potential harm to residents, staff, and
damage to property.
• Conduct apartment inspection and report any repairs.
• Answer phone calls and write down appointments for clients as well as collect and sort clients mail.
• Answering fire safety alarms in building when incident occurs and relaying information to FDNY.
