-Telephone triaging patient symptoms, questions and concerns. Collaborating with physicians and mid-level providers and directing, communicating and providing recommendations for individualized patient treatment plans.

-Providing patients with abnormal test results and physician recommendations.

-Facilitating and thoroughly educating patients on cardiac procedures and referrals to specialists.

-Performing nurse visits for medication education, blood pressure and heart rate checks, and assessing cath sites.

• Provide nursing support to 5 or more patients on Telemetry, Progressive Care and Medical Surgical floors
• Assess and document data related to patients care
• Develop, implement, and evaluate plans of care
• Educate patient and family on disease process, medication, treatments, and safety measures
• Precepting new employees
• Occasional case management duties

• Provide and complete comprehensive nursing health assessments to assist in identifying and resolving clients health and safety concerns
• Ensure medication is properly administered and monitor for effectiveness and any side effects/adverse reactions of medication
• Provide frequent guidance, support, and recommendations
• Adhere to standards of care/treatment plans according to a person centered approach
• In cases of individuals with complex medical needs assist in developing community supports and contacting healthcare providers on their behalf
• Provide periodic review concerning health care treatment
• Monitor and evaluate medical charts, nursing care guidelines, health care plans, treatment and response as outlined in the Person Centered Plan (PCP)
