Provide Speech-language pathology services to in-patient rehab and LTC residents. Perform assessments/evaluations, develop
personalized POCs/treatment plans, participate in IDT care plan meetings, participate in concurrent/collaboration treatment with OT and PT, and provide ongoing family/staff education and instruction.

• Created/built in-house rehab team from start in August 2022. Lead rehab department and am an integral part of the clinical rehab team. Active participation in utilization review, IDT care plan meetings, clinical review meetings, leadership engagement, and hiring. Provided SLP services to in-patient residents.

• Lead rehab department and managed rehab program in the facility. Active participation in utilization review, hiring, case management for managed care, and mentored a CF.
