• Collaborates with multidisciplinary teams, patients, and community agencies to ensure continuity of care and optimal patient outcomes.
• Responsible for comprehensive care coordination and management of patients with complex medical needs,including assistance with transitioning to post–acute care settings.

• Work with sexual assault victims, domestic, elder, and child abuse victims, patients with chemical dependency and other pyscho-social emergencies, patients needing nursing home placements and general resources such as transportation and housing

• Collaborated within a multidisciplinary team to optimize treatment approaches and address psychosocial needs of patients.
• Conducted comprehensive assessments to identify patient needs, ensuring tailored interventions and resource allocation.
• Facilitated support groups in order to address the emotional needs of both patients and their caregivers.
