  • Responsible for planning, developing, and implementing the comprehensive care roles within Comprehensive Care Management, and interdisciplinary departments to facilitate safe throughput practices.
  • Accomplished multiple tasks within established timeframes, formation of MDR workflow, ELOS workgroup, Readmission Council & Employee Experience.
  • Built a resilient team of RN Care Managers, Social Worker's and Health Coordinators and maximized performance by monitoring daily activities and mentoring team members. And there for increasing teams agility and performance.
  • Improvement of People Survey response in domain of Culture of Safety from 75% to 90.6% from FY23 to FY24. Improved favorability in the domain of Burn out from 60% to 87.5% from FY23 to FY24.

  • Multidisciplinary Rounds initiatives, reformulated back to basics best practice among multidisciplinary team members. Which included, training, access to tools and establishing accountability amongst all disciplines.
  • Multidisciplinary Rounds attendance increased from 22.5% to 86.6% and documentation of Extended Date of Discharge (EDD) from 49% to 94.5% in FY23. Maintained the same performance YTD FY25.
  • EDD and discharge timeliness increased from 51.7% to 95.6% in FY23. Maintained the performance through FY24 and YTD FY25.
  • Initiated a Extended Length of Stay workgroup for the Hospital. Comprising of Providers, Senior Leadership and Multidisciplinary team members to address barriers and achieve Length of Stay Operational goals.
  • Achieved reduction of ALOS-Adult performance from FY22 at 4.42 days to 3.94 days by close of FY24. 0.48 day reduction in 2 fiscal years and maintaining the same as of FY25.
