Providing Intense Case Management

Providing direct vocational rehabilitation services to disabled Veterans. Meeting with Veterans at their homes. Interviewing Veterans. Assessing Veterans needs and a progress of their individual rehabilitation plans. Providing analysis, recommendations and comprehensive reports to VAMC facilities in Indiana.

Contracting with the Matrix Companies to provide Womer’s Compensation case management to the

injured workers in Indianapolis

Meeting directly with the clients and interviewing them at the doctor’s offices during their appointments. Participating at the appointments and collaborating with the medical professionals to insure the best health outcomes for the injured workers. Assessing and analyzing clients health conditioning order to provide recommendations. Completing required paperwork and writing comprehensive reports to Matrix Companies. Staying on top of the latest medical research.

Worked directly with adults with mental and physical disabilities. Provided comprehensive vocational services such as interviews, assessment, research, review and analysis of medical data, planning, job search & placement, referrals, follow up and other relevant rehabilitation services. As a Senior Counselor, approved my own IPEs and IPEs of other Vocational Rehabilitation Counselors when was asked by Office Supervisor. Served as Acting Office Supervisor upon request. Served as a Liason with community agencies.
