• Maintains all responsibilities as stated for Referral Clinical Coordinator I.
• Collaborates with hospital physicians with the administration of DCD order set after authorization has been formalized with family members/authorizing parties.
• Updates all labs and imaging necessary for donor work up and eventual allocation.
• Monitors donor’s hemodynamic stability, lab trends, and cultures, notifying the physician of concerning vitals/labs and collaborating to order appropriate interventions necessary to maintain organ suitability.
• Reports donor to CME, providing all documentation needed for release of organs and tissues.
• Collects serologies and HLAs according to One Legacy guidelines, including notifying the receiving labs of their estimated arrival.
• Secures pronouncing physician and schedules OR times once donor is in the final stages of allocation.

• Performs referral triages, collecting clinical and social information necessary to determine initial organ suitability and family dynamics.
• Conducts referral site visits at any of the 200+ hospitals under One Legacy’s donor service area.
• Evaluates medical records for current organ function, discussing concerning derangements with One Legacy supervisors and Referral Management Medical Director to formulate a plan of action on how to proceed.
• Utilizes AAN guidelines when reviewing brain death documentation, and communicates with authoring physicians when revisions are needed.
• Collaborates with hospital staff to ensure the opportunity for donation is maintained
• Provides real time education to nurses, physicians, social workers, etc. on policies and procedures including:
• Clinical triggers that would necessitate a patient to be referred to One Legacy
• The process of approaching families, and how to do so appropriately so as to maintain hospital propriety

• Provided transportation to and from school, doctor's appointments, etc.
• Engaged in age and ability appropriate enrichment activities to promote child development.
• Prepared meals according to children's specific dietary needs.
• Applied conflict resolution techniques to deescalate emotional distress/conflict between children.
• Complied with state and federal child care provider guidelines and regulations.

• Trained in the care of children 6 months to 18+ years with varying physical and mental disabilities.
• Follows all instruction and advice given by nurses and PCAs, consistently communicating in order to provide
• Proficient in g-tube feeding administration and care.
• Maintained accurate documentation of medications, vitals, feedings/meals, and general intake/output for each
• Monitor clients for risk of severe medical complications and provide appropriate medical interventions.
