2010-2014: Chemical Health Case Manager doing intensive case management for individuals with chemical health and mental health needs. Completed MICD Commitments. 2015: Elderly Waiver Case Management-Case manager for individuals over age 65 to ensure all health and safety needs were met while living in the community as independently as able. 2016-Present: MnChoices Reassessor-assess individuals on the CADI, CAC, DD, EW, AC waiver for health and safety needs using the MnChoices Tool. Make recommendations for services that can best meet their needs to remain safely in their home. 2024-Present: Crisis Response Team: Respond to crisis calls over the phone and in person in the community. Collaborate with community members to ensure needs are met.
