  • Administered medications safely according to established guidelines while closely monitoring for side effects or adverse reactions requiring intervention. As a med aide, Assisted with monthly changeover, worked on med cart with narcotics for end of life cares excedra.
  • Utilized critical thinking skills to prioritize interventions based on residents'' acuity levels and individual needs. Ensuring ADL's are met, Based on diet, and working with speech and OT for best swallowing route for med admissions.
  • Aided to identify changes in residents" condition, promptly notifying charge nurse, DON and initiating appropriate interventions instructed by charge nurse/DON/Physicians when necessary.
  • Trained CNA/TCA on cares for residents, from handwashing, oral, peri cares, positioning. Educated them on care plans, gait belt usage, hoyer lift and when to notify their nurse.
  • Worked with "non emergency" and "emergency" transfers to ER commuicated with family and POA's on changes to residents. Worked with hospice nurses around the clock with changes and suggestions for hospice cares.
  • Prominent in Point click Care, POC, MAR, TAR, trained other staff in watching for med changes in MAR (some medications need BP, Pulse, or O2 stats taken to administer meds safely.)
  • Collaborated with all staff to quickly assess residents and deliver appropriate treatment instructed by my charge nurse or DON while managing rapidly changing conditions.
  • Made sure to sign and initial everything in communication logs to make sure no room for errors with the delicate community we cared for.
