Prescreened and interviewed potential clients as well as gave tours to both them and other agencies. Completed all new Intakes and ASI assessments. Created new contacts and tracks as well as tracked all new clients in Salesforce and HMIS database. Provided individual Case Management weekly with clients who where in the intern phase as well as all clients who were transitioning out into housing and graduates for up to a year upon completion of the program. Provide resources for getting documentation to gain employment for the clients on my caseload. Housing search with clients including setting up tours, coaching around barriers they may have to address and providing resources for building credit. Ran both male and female Seeking Safety and Life Skills groups. When applicable submitted applications for the Aurora Flex Funding for clients.

Support graduates in the community one year after graduating the program. Check-ins weekly, bi-weekly and monthly by home visits in the graduates home setting or phone calls or in office meetings. Assess and act accordingly for crisis interventions with graduates in crisis mode. Deliver food boxes or provide valuable resources to address issues or needs that graduates have. Create monthly events for graduates to stay connected to the program and to keep coming back. Coordinated getting graduates to annual events. Track all notes in the Salesforce database for all interactions with clients.

Assessed individuals' needs and planned bespoke recovery programs to achieve personalized goals. Set up and adjusted visual aids to help pilots make safe landings. Tracked client participation in treatment services to identify attendance issues. Improved relapse prevention rates with targeted recovery program mentoring.

Provided transportation for the trainees to and from the contracts. Worked alongside the trainees as their supervisor but training the skills needed for each contract through leading by example. Kept logs for the different jobs required per each contracts as well as daily reports for the case management team about what occurred and any concerns related to the trainees I had on my crews. Outreached for potential clients and contracts for the BridgeHouse program. Performed daily safety meetings, weekly vehicle checks and reported as needed for maintenance issues. Used De-escalation and mental health first aid on site whenever issues arrived amongst clients on the contract. Coordinated appropriate care whenever needed for relapse based on severity.
