As an RN, I am always scanning assessing and performing ADPIE skills. In the hospital as an LPN, we were allowed to assess patients. During that time, I learned how to assess very quickly and accurately to provide the best care for them. I was assigned 22 patients in a long term care/ rehab setting. Time management was of the essence and was demonstrated daily as we worked 12 hour shifts and were responsible for 3 med passes daily.

We work together and huddle daily to make sure that everything is getting taken care of throughout the day. Also, I take care of the needs of the veteran's to include if they need follow up or specific needs all in one visit so they don't have to make multiple trips and saves the veteran time.

- I always try and start conversations with employees and staff to ensure comradery and keep moral high. If there is a process that needs feedback by other staff, I let them know in a nice way of how to fix it for next time. I like to use the teach back method as this helps them repeat what they have learned and form an understanding.

- I always try to support colleagues and other nurses by educating or counseling them if they have a problem. If they are sick or have high blood pressure I check there vitals and assist them with their needs.

- Working as a PACT RN in training, it is imperative that I document every detail of the encounter with the veteran so that the provider and other staff have the best picture of the veteran to allow for the best care. For example, a veteran needed cardiology referral. As a Pact RN, I made sure that they had all of the information in there in order to receive the consult. After just getting out of the hospital for a cardiac event it is important to follow up with the veteran to confirm they plan to go there and have made an appointment. Once I do that , I also confirm with the cardiology office and make sure they have all diagnostic testing needed in order to obtain the consult. If I were not to follow up on this or leave something out it would delay care. That is why I set reminders in the tracker system to ensure the best care for the veteran and make sure there is follow up with the veteran.
- PACT program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.
- Monitoring diabetics, hypertension, chronic disease, preventative care, access to care, patient satisfaction, my healthy vet accessibilty, vvc/cvt accesibility, reduction of veteran going to the ER by providing appropriate follow up based on chronic disease and sensitive conditions, and management of panel based off size and acuity, timely follow up regarding transition to care, team huddles to support team communication and team work to ensure consults are being completed appropriately.

- I use ADPIE daily to make sure I am assessing the veteran , going off of the provider diagnoses as well as forming a nursing diagnosis such as the health assessment data to identify current health problems , Planning a care plan for an individual, Implementing nursing interventions to take care of the veteran, evaluating the veteran to see if the implementations have been affective.

- When a veteran has a blood sugar of 600 and goes AMA in the hospital and shows up at the clinic, I make sure that the first thing I do is , check to see mental status, obtain a POC glucose from the pcp, check the glucose, and alert the pcp to do form a plan by seeing him. Educating the veteran of how to use a blood glucose machine and making sure the veteran has access to appropriate resources and medications to manage diabetes.
