Plans, organizes and directs short- and long-term hospice care to patients with an array of hospice patients. Responsible for supervising multi-disciplined teams, administering medications, symptom assessment and management, education/teaching, and end-of-life care.

Conducts patient assessments, coordinates the care team to devise and execute care plans, and facilitates referrals to other disciplines as necessary. Promotes patient health via education and rehabilitative measures, guiding patients in adopting self-care practices. Supervises LPNs and HHAs, maintains ongoing communication with the clinical manager and care team and conducts staff education initiatives.

Develop and implement written care plans with an Interdisciplinary Team (IDT). Evaluate and change plans as necessary to meet patient/family needs. Responsible for the direction of care provided by LVN and HA across San Diego County. Provide clear communications to IDT and primary MD. Educate patients and family members. Orients, instructs, and trains assigned personnel.

Manage the direct patient care for all infusion outpatients actively undergoing treatment. Assume responsibility and accountability for the delivery of all patient care, to include but not limited to education, administration of chemotherapy, blood product transfusions and other supportive care infusions.

Promote and restore patient’s health status; collaborate with physicians and multidisciplinary team members; provide physical and psychological support to patients, family members and significant others; and supervise assigned team members. Uses a systematic critical-thinking to guide assessment, nursing judgment, and evaluation of human responses to health problems through activities such as health teaching and the provision of care.

Serve as a patient advocate by promoting their physical, spiritual, and emotional well-being. Educate patients, administer prescribed medications and treatments, and develop care plans based on patient needs.
