• Perform various OT services including evaluation, recertification, discharge, and progress note documentation as well as various treatment and intervention sessions
• Manage a daily case load of 6-8 geriatric patients at Rehab/LTC facility including MCR A, MCR B, Managed Care, and VA patients
• Evaluate, document, and establish ADL/IADL/Orthotic/Wheelchair positioning goals for geriatric patients of varying diagnoses through collaboration with multidisciplinary teams for treatment development and implementation
• Implement various interventions to increase strengthening and activity tolerance to improve ADLs including therapeutic exercise, therapeutic activities, neuromuscular reeducation, sitting/standing tolerance activities, work simplification techniques, physical agent modalities, and self-care management
• Consult with COTAs concerning different patients' diagnoses, goals, treatments, and interventions
• Working knowledge of with Medicare ICD10 codes.

  • Manage a daily case load of 1-2 geriatric patients. (Home Health)
  • Evaluate, document, and establish ADL goals for geriatric patients of varying diagnoses. (SNF/Home Health)
  • Implement various interventions to increase strengthening and activity tolerance to improve ADLs including therapeutic exercise and self-care management. (SNF/Home Health)
  • Consult with COTAs concerning different patients’ diagnoses, goals, treatments, and interventions. (SNF only)

• Perform various OT services including evaluation, lifestyle addendum, plan of care, and discharge documentation as well as various treatment and intervention sessions
• Manage a daily case load of 6 clients plus a group of 2-6 patients
• Evaluate, document, and establish ADL goals for patients of varying diagnoses
• Implement various interventions to increase strengthening and activity tolerance to improve ADLs including therapeutic exercise, therapeutic activities, neuromuscular reeducation, sitting/standing tolerance activities, work simplification techniques, physical agent modalities, and self-care management
• Consult with COTAs concerning different patients' diagnoses, goals, treatments, and interventions.
