• Assisting the nursing team by writing reports and charting injuries/falls, and helping to evaluate patients with behavioral concerns or a change in medical condition. Recording vital signs, and administering medication.

• Providing direct care for individuals that require skilled attention.
• Supporting the care-team and assisting patients as they progressed into end-of-life care.
• Helping with activities of daily living, promoting good oral and personal hygiene by aiding patients with shaving, bathing, and teeth brushing
• Answered call lights and supporting patient comfort and safety, by assisting with ambulation, feeding, and toileting assist.

• Providing comprehensive care in a fast-paced; team-oriented environment
• Assisting up to 40 residents: either in our assisted-living wing, or in our Memory-Care.
• Where I helped residents with activities of daily living, self-care, and passing meds.
• Direct care, management, and support of COVID19 critically ill patients, various oxygen management, and diabetes (BG) management.
• As well as feeding-tubes, and tracheostomy care.
• Skills employed include physical assessment, medication, checking and maintaining BG levels (for type(s) 1, & 2 diabetics) and administered insulin as needed
• Tracheostomy care- trained by the careteam: to clean, suction, & manage emergencies
• Provide hospice care to those in need; as well as assisting with the transition to end of life care.

• Provided round-the-clock care for seriously ill family member -dealing with Vascular Dementia, and Parkinson's'-, including medication management, assistance with activities of daily living, coordinating in-home therapies and services, and therapeutic recreation.
• I was fortunate to have the health care skills to be effective in that difficult role, while learning a lot about skilled nursing; as she progressed to hospice care, and eventually passed.

• I worked at a fast-paced care facility - in both their Assisted Living, and Memory-care
• Assisting residents with activities of daily living, oxygen care/maintenance, as well as assistance with feeding tubes, and feeding residents; who were no longer able to feed themselves
• With my self-leadership, and ability to communicate -effectively- with the team, I rose to the level of peer-mentor, and started training new hires; and helped with their onboarding experience
• Helped residents who had Hoyer-lifts, and helped re-direct, and care for those with extreme dementia.

• Providing high quality care to patients in their homes: assisting with dressing, grooming and feedings needs, helping to overcome, and adapt to mobility restrictions.
• As well as assisting with medication reminders, ROM exercises, and helping them drive to/from appointments.
• Helped monitor/maintain vitals - through exercise, and proper care.
