HBPC Case Manager: 2006-present

• Provides medical management in the veterans home, in the primary care setting. The RN Case Manager on the Home Based Primary Care team provides health prevention and wellness, as well as support and health education to both the veteran and his/her caregiver/family members.
• By using the nursing process and evidence-based practice works collaboratively with the HBPC team, which includes- Provider, Physical Therapy, Occupational Therapy, Pharmacist, Registered Dietician, Psychologist, Social Worker, and the veteran.
• Plans, assesses, implements, documents and evaluates care for the veteran with a focus on self management, prevention and wellness using holistic approach to achieve the veterans health goals.

Primary Care: 2001-2006

• Evaluated nursing practices in the primary care setting to identify areas for improvement, implementing evidence-based practices to improve patient outcomes.
• Assisted physicians during examinations procedures by preparing necessary equipment supplies providing direct hands-on assistance when required.
• Coordinated and taught Diabetic Education in a classroom group setting with veterans.
• Provided care for patients with chronic conditions such as diabetes, hypertension, and heart disease through regular monitoring and medication management.
• Assessed health status of patients by completing health histories, physical assessments to develop plans of care.
• Facilitated effective care coordination among multidisciplinary teams by collaborating with primary care providers, specialists, social workers and other team members to ensure a comprehensive approach to patient care.
