• Initiates and maintains communication and collaboration with physicians, social workers, care team leaders, staff nurses, other care giving disciplines, and patients/families to develop, implement, and evaluate a transition plan of care for each patient. Serve as consultant and in-service leader in multiple medical departments (psych., physical therapy, occupational therapy etc), for the transition from hospital/rehab, to homecare.

• Visiting patients in homes, taking vital signs, setting up Bluetooth equipment so the patients can take own vitals daily.

• Arthritic exercise routines daily with Alzheimer's and dementia patients, mental and physical stimulation activities provided daily, such as: bowling, word games, corn hole, bingo etc. Reading to patients in hospice care, one on one room visits, weekly and monthly care plan reviews, and goals. Occasionally helped patients with dressing for the day, and maintain living spaces.

• As a companion aid, I would help provide emotional support, caregiver assistance, light housekeeping, and companionship to help senior and elderly adults function independently in their own home environments.
