• Provide leadership and accountability for the performance of a team of Advanced Practice Clinicians (APC’s), Registered Nurses (RN), Clinical Advisor (CA). In collaboration with the DCO, I am responsible for clinical outcomes and business operation targets of market.
• Primary Responsibilities: Directly manage clinical staff. Set team direction, resolve problems, and provide guidance to members of own team. Set priorities for care team by monitoring trends, relationships, and outcomes. Ensure collaboration between all team roles through direct supervision, coaching and influencing. Ability to collaborate with multiple team members and pivot to team needs. Ability to communicate effectively to multiple market leaders. Manage salary, bonus incentives and policies in a manner which reward performance excellence. Ensure effective orientation and development for clinical staff. Utilize performance tools that hold the clinical team accountable for market metrics and performance standards. Promote individual development by providing learning and growth opportunities to clinical staff. Develop solutions to problems or barriers by partnering with key stakeholders. Serve as a role model to internal and external partners.

• Responsible for providing mentoring, coaching and support for Advanced Practice Clinicians (APC) and Registered Nurses (RN) in the field. Acted as the lead resource supporting advanced illness management and advance care planning for the clinical team. The CA reports to the Clinical Services Manager (CSM). The CA partners with both the CSM and or Director of Clinical Operations (DCO) to enhance clinical expertise and adherence to Optum’s ISNP clinical model. Primary Responsibilities: Set team direction, resolve problems, and provide guidance to members of their clinical team. Support CSM.

Fee for service SNF, ALF, IL, STR visits for primary care physician in seven centers in Pinellas county.

Conduct comprehensive assessments. Effectively manage acute and chronic medical and behavioral conditions. Ensure accurate and complete documentation of acute and chronic medical and behavioral conditions. Communicate and collaborate with interdisciplinary care teams. Conduct quality advanced illness and advanced care planning conversations. Provide members and caregivers with counseling and education. Utilize advanced clinical nursing expertise, knowledge of geriatric/chronic disease management, and the long-term care industry to provide best in class care to members. Anticipate customer needs and proactively develop solutions to meet them.
