With over 15 years of experience in geriatrics, I have developed a specialized focus in home health, utilization management, ICD-10 coding, and acute care within Skilled Nursing Facilities (SNFs) in Modesto. I have successfully managed the care of 1-15 patients at a time, ensuring safe discharges through comprehensive patient management. My responsibilities encompassed conducting detailed history and physical exams, coding services and diagnoses, ordering labs, managing narcotics and routine medications, and collaborating with multidisciplinary teams to facilitate safe patient discharges.
