• Responsibilities include: Direct oversight of the Referral Office staffs (12-15 Intake Assistants and Intake Coordinators); Staff scheduling; Performance, productivity and quality reviews; Evaluation and refinement of office procedures and processes; On call rotation monthly and as needed; Act as a resource for all admission teams as well as referral sources.

• Responsibilities included meeting new patients and families, determining eligibility for hospice services, initial assessment, and symptom management. Additionally, I serve as mentor/preceptor for new team members on the Assessment Team and for the agency in general.

• Responsibilities included phone triage and coordination of visits by field staff.

• Serving the inner city and surrounding areas. Patient populations included, Adult Home, AIDS, Pediatric and Alternative Home teams. Responsibilities include pain and symptom management and identification of specific educational needs in order to provide the best care possible for the patient and family.

RN: Responsibilities included initial assessment and triage of patients, telephone triage, provide patient education on illness/injury, medication administration,vaccine , administration of basic hearing test.

• Provided care to children with cancer and their families in the ambulatory setting. Responsibilities included administering chemotherapy and blood products, management of central lines, and administration of moderate-deep sedation, management and recovery of the sedated patient, patient education and telephone triage. Served as a resource for medical/nursing staff regarding hospice and palliative care issues and assisted with management of clinic (flow of patient traffic and billing). Additionally, served as a volunteer for Camp Friendship.

• Responsibilities included providing holistic hospice care to children and their families as part of the transdisciplinary team. Duties included home visits, on-call rotation, bereavement follow-up services, program development, mentoring students and continued staff and community education. I have spoken on issues regarding hospice care at local, state and national levels.

• Provided care for children with a variety of acute and chronic illnesses. Served as a core member of the Children’s Hospital Brain Tumor/Oncology Support Group.
