I listen to report from my charge nurse about morning shift tour then switch over to bedside report from morning PCT

Next, I log into the computer set up my assignment then grab my accu check machine go room to room and get my night vitals.

I assist patients to the restroom, provide bed changes, pick up trays, pass out morning lines, give 4 am baths, pass out snacks, help set up rooms for new admissions, and help back up patients' belongings.

  • As a Certified nursing assistant each day changes with our patient ratio and their care needs. i start my night off with report from my night charge nurse on all the care needs and detail about our patients. once we are given report from her it is our morning aides that walk with us and give us bedside report on the current patients and the one's that gave been discharged. From there we get on our computers check off all the patients on our list made by our charge nurse begin vitals, and blood sugars, intake and out puts. Carry out tasks such as obtaining answering call lights, answering the phones, send faxes, take inventory, file patients charts enter information in patients charts under Micro Soft Word, CPRS and VISTA, light housekeeping, usage of equipment maintaining cleanliness and making sure equipment function properly or was tagged appropriately
  • Vitals are taking as soon as the patient arrives on floor, blood sugar if needed the patient skin is checked and he or she is given a gown and their checked for cleanliness if our patient needs help with gown or they are incontinent we work as a team and get our patient cleaned .changed and turned
  • New admits, gather the patients vital and weight, charting all their belongings perform MRSA swabs upon admission and discharge, assist with discharges gathering all the patients' belongings, assist the patient out of the facility with their belongings, assist with stripping the room to be cleaned for next patient, set up room suction canister plus tubing
  • when a new patients arrive up stairs we help transfer our patient and we make sure the beds are pushed close together. Once their monitors and any catheters, nasal tubing, and any jp drains are made clear we roll the patient side to side and roll a glide and bed pad under patient for a smooth transfer both beds are locked and we pull on the count of three then a full skin check is done by nurse with our assistance.
  • When it comes to our patient hygiene after reading our charts ,we talk with our patient and let them know their bath times and ask what kind of assistant do they need help with. Some patient are still independant and require minimum assistance while others are total care.
  • Provide the patient with personal hygiene items, ice water pictures, assist with bathing and grooming, as well as set ups for bathing for independent patients, provide fresh linen for care and for the independent pts
  • We assist with transfer the patients around the hospital when needed and help the to them outside to their transportation home. We lock their wheel chair brakes stand beside them and lead them to the seat in their vehicle while their family gathers their belongings but if the patient is weak we take a gait belt place around patient and slowly turn them to the seat in their car.
  • Provide escort to and from therapies, doctor appointments inside of facility, perform ROM
  • Help pass nurishment, performing calorie counts at meal times, assisting with intakes and outputs of the patient's fluid consumption, urinals, commodes
  • Encourage the patient at meals, bathing and grooming, assist patients to and from bed, wheelchair, and restroom with the proper lift equipment
  • we collect urine and empty each one in a cylinder and document the out put, they may have jp drains and we collect in separate contains to keep accurate measurement.
  • Insert and take out catheters, perform bladder scans, EKG, take IV's out, set up Telemetry box on pt
  • Move beds, work on different floor to assist with needed staff.
