Ensuring on time 485 recertification reviewed and with Physician signatures, for all home health clients.

Maintaining all client medical records up to date , organized and ready for State Inspection at any time.

Review of all admissions, transfers and discharges to ensure they meet policies, procedures and standards of practise.

Monitor client progress or decline, co ordination of services, expected outcomes and participate in MDT and / or family discussion on appropriate level of care.

Present accurate data reflecting Nursing audits to our quarterly board member QA meetings

Providing guidance and support to C.NA Manager and C.NA team with 24 / 7 on call availability.

Training and delegation of appropriate tasks to home health C.NA team, with quarterly follow up, reviews and education refreshers.

Creating and presenting regular in service sessions on relevant topics for our home and community based services.

Providing annual wellness checks for employees, including TB skin tests. Assessing and completing any accident or incident reports for employees and clients as needed.

RN cover for Arcadia Adult Day Care and Day Health center as needed.

Interviewing of potential new hires.

Onboarding and training of all new hire clinical employees.

Provided HMSA members with support, education and resources to assist them with positive recovery and improvement of their health. Promotion of self management to those patients with chronic health conditions or as identified by HMSA predictive model.

Guidance and co ordination of care for patients with complex health conditions to connect them with care, equipment, treatments, transport and other required services.

Working with physician groups and families to provide the best possible outcome for their loved ones.

Pre and post op care for acute surgical patients.

Providing assessment, pain management, infection prevention and facilitating positive recovery.

Care of medical patients with chronic conditions. Providing patient education and advocacy.

Experienced and skilled with multiple medical devices, not limited to; IV medications, oral medications, PCA, PICC lines, G tubes, NG tubes, oxygen delivery and nebulizer treatments. Urinary catheter insertion and care, wound drainage systems, pressure relief devices.

Frequent float duty to Telemetry, ER, ICU and post partum floors.

Regular code blue responsibilities.

Supervision of new nursing staff and student nurses.

Responsible for the intake and assessment, care and education of patients pre, peri and post various same day procedures.

Close monitoring of sedated patients.

Assisting the Physician with bedside procedures; Endoscopy (gastroscopy, colonoscopy, sigmoidoscopy, T.O.E's, Bronchoscopy, Cystoscopy and ERCP ) obtaining gastric and colonic biopsies, removal of polys and extraction or destruction of stones.

Assist with supra pubic catheter placements, liver biopsies and pleural aspirations.

IV blood transfusions, venesections, venepuncture, varied endocrinology investigations.

Set up and monitoring of patients infusions and injections of cytotoxic medications.

Supervision and support to student nurses and new hire RN's.
