Provide medical care including med pass and sick call to all ICE detainees on Angola State Penitentiary campus.

• Responsible for visiting patients homes in the community to provide wound care, medication education, physician outreach, and make sure patients spend more time in their home adhering to md orders while preventing frequent hospitalization. Mandatory daily documentation required to maintain state compliance for record keeping. Requires meticulous time management to ensure patient satisfaction and timely care to patients while in the field.

• Responsible for providing in class and clinical instruction for CNA students and preparing them for certification test. Clinical instructing is my main focus.

• Responsible for locating data to close care gaps for members across various EHR systems. Required accuracy and detail to ensure that all gaps would be closed in the measure year. Close contact with providers and outreach with other clinical staff required to ensure that all gaps were closed timely with accuracy.

• Responsible for working in the field as a care manager to make sure that transitions of care for members were documented through a designated program provided by Humana while working remote. Phone outreach required to speak with members and schedule in home visits with members to assist with closing care gaps, especially for HEDIS measure TRC. Accuracy required when entering records to ensure gap closures.

• Responsible as the Team Lead LPN to make sure that all 4 elderly group homes had 2 med passes performed daily, 7 days a week. Responsible for hiring age scheduling nurses to ensure med passes were performed in a timely manner due to nurses having to travel several miles to get to each home. Maintain accurate MARS in homes and responsible for all medication ordering.

• Duties included supervision of 12-18 nurses and CNAs per shift. Worked closely with DON and ADON to ensure residents were taken care of per DHH standards. Assisted with managing schedules, state surveys, responding to denial letters from insurance companies, and providing reasons to insurance companies why the nursing home should be reimbursed for services rendered.
