  • Upon assignment of a new client, proceeding within the required guidelines, the Client Care Coordinator is to immediately contact the client, and set an appointment to meet with the client at a CHOICE office or off-site to complete all required paperwork
  • For all assigned cases: Set client goals and align care activities in accordance with these goals
  • Monitor the progress of goals, and adjust care plans accordingly
  • Collect and enter data and notes accurately, thoroughly, and timely, documenting activities and outcomes into case management systems
  • Plan and execute care activities in accordance with client goals to minimize any potential issues with billing and reimbursement to the agency
  • Plan and utilize time so that needed outcomes for the client are achieved and completed within the time frame set by the Supervisor or Government agency
  • Proactively communicate with all members of a clients care team
  • This includes Choice of NY Director and Supervisor, Primary Care Provider, Mental Health Provider, Probation Officer, Clients Attorney and Supervising Court
  • Report on any emerging issues and needed adjustments
  • Connect with each assigned client minimally 4 times per month
  • Review assigned caseload with Care team 2 times per month to ensure effective case management while making adjustments as needed
  • Develop and maintain productive and results-oriented relationships with mission-critical persons at outside organizations
  • Attend meetings, as necessary, at the Department of Community Mental Health, local courts to review client cases and progress
  • Using the full capacity of one’s ongoing experience and training, demonstrate the progressive ability to problem solve, advocate, mediate and handle increasingly complex tasks related to care coordination
  • Meet one-on-one with the person needing services, and screen to determine eligibility
  • Conduct intakes at the Westchester County Jail and courts within Westchester County
  • Track activities and outcomes
  • Administering risk and needs assessments while assisting clients with their job search and connecting them to job training, legal advocacy and accessing public benefits
  • Providing assistance with housing applications and searches
  • Transport clients from County Jail upon their release.

  • Facilitate group and individual sessions to support incarcerated women and men with processing/unpacking identified concerns, addressing symptom management, and working towards solution based outcomes
  • Implement the application of psychoeducational models, trauma-informed interventions, and other effective approaches /best practices for addressing the needs of incarcerated individuals
  • Link incarcerated women and men to community resources by supplying referrals geared towards helping participants focus on their mental health, housing, family, employment, or any other life management barriers that could potentially increase recidivism
  • Assist with transitional case management to prepare incarcerated individuals with re-entering into the community setting
  • Participate in weekly Core meetings to present insight about the C.O.R.E program and its effectiveness and execution of current services that are being rendered.
