I oversee a Residential Group Home of four individuals with IDD. I provide direct support to the individuals including assistance with daily living skills, medical appointments, medication administration, and anything else that the individuals need or require. My responsibilities also include communicating with medical professionals for each of the individuals, completing schedules to ensure accurate coverage for the program, monthly Treatment Team agendas as well as facilitating the meetings, monthly site inspections and paperwork to ensure compliance with regulations.
I provided assistance to 8th grade students in Learning Support classes at the Mountain View Middle School. I collaborated daily with the classroom and the Learning Support teachers to foster independence, time management skills, direct assignments, as well as homework and testing support while following each student's Individualized Educational Plan.
I provided supervision and direction over four residential programs serving individuals with intellectual and physical disabilities while assuring compliance with state regulations, Federal Waiver, and HUD requirements as necessary. I provided coverage and direct care to the individuals in the programs including, but not limited to, assistance with personal care, daily living skills, medical appointments, medication administration, and behavioral support as needed. I completed quarterly and annual assessments for each individual as well as communicated with the individual's county Supports Coordinator to ensure that their Individual Support Plan (ISP) was accurate and updated as well as keep the Supports Coordinator informed of any concerns or information needed. I completed Certified Investigations as assigned and trained staff in Medication Administration.
I had a case load of approximately 30 individuals, all who were receiving vocational training services at the Pollock Center for Industrial Training (PCIT) division of CPARC. I completed assessments and quarterly reports for each individual, facilitated quarterly meetings to discuss progress, growth, and goals, and assisted with behavioral support as needed. I communicated with the county Supports Coordinator to ensure that the Individual Support Plan was accurate and updated and informed them of any concerns or information as needed.
I supervised approximately fifteen individuals in a work group at the Pollock Center for Industrial Training. My duties included teaching the trainees how to complete the different jobs that PCIT contracted for; assisting them in learning the jobs, retention, quality, and other skills such as attendance and punctuality. I completed documentation for each individual's progress, growth, and goals as well as reported on these at the quarterly meetings.
I supervised the Community Support Program, a residential program that provided habilitation services to individuals either living at home with their families or in a nursing home setting. I created schedules for six staff and approximately 20- 25 individuals receiving the services as well as provided direct support to the individuals in the program as well. Completion of monthly billing was also one of my responsibilities and monthly meetings with the individuals, families, and Supports Coordinators.
Provided direct care to two individuals that lived in their own apartments. Assisted with daily living skills. personal care, medical appointments, medication administration, and behavioral support as needed.
