Providing Primary care to Bay Area residents using the One Medical model of care.

• Supporting the Diablo District in the NorCal Region which includes 4 locations including San Ramon, Dublin, Walnut Creek, Concord, Berkeley University and Berkeley Shattuck locations.
• Oversee day-to-day practice functions, including delivery of patient care, medical and administrative staff and business activities.
• In charge of the growth and development of the District and meeting Model Office metrics and Providing exceptional Clinical Quality Metrics.
• Support Access for Norcal Region; Initially oversaw Casual (Per Diem) providers and supported providers who were IOP's transitioning to a fully remote role.
• Led a multi disciplinary team to help create much needed access for the Norcal region by leveraging the availability of underutilized SoCal Providers.
• Helped develop the Remote PCP role, including the playbook, hiring standards, interviewing and hiring of initial providers, communication to all teams and leaders involved, and creating and leading the onboarding. Facilitated clinical and leadership support while transitioning into the new role.

• Supporting the providers at the San Ramon and Dublin sites to provide care that support the patient needs, guide them in their professional growth and get them on track to meet the requirements to be a STAR provider
• Partner with the office manager at both the Dublin and San Ramon sites to help in overall office operations, office clinical flow, communications to staff, leading huddle, engaging our teams in projects, and making sure we are meeting all of the Quality and Value metrics

• Provide Direct Patient Care and model key Crossover attributes of servant leadership, integrated collaborative care approach, following Crossover standards, presenting clinical topics in huddles, and delivering patient care and clinical outcomes.
• A clinical resource to the care teams, helping manage patient experience (feedback, complaints and resolution) and supporting the FB internal page manager for clinically related posts,questions, and periodic bulletins.
• Supervisor to 11 medical providers
• Assist with Recruiting, Hiring, Training, Retention, and direct management of Life@Physicians. This included helping to manage daily schedules, resource physician of the day, PTO requests, CME approvals, and related physician management issues, including
  annual performance reviews

  around physician specific issues, meeting followups, calendar of events, and related news items to ensure that all the physicians are informed, have a voice at the practice,and generally leading the physicians to higher performance, engagement, and results.
• Partner with the Nurse Lead(s) to support Clinical and Practice Operations including updating EHR workflows as well as managing dispensary medications, vaccines and DME supplies.
• Lead the Practice in Quality Improvement Programs, as well as participate in multidisciplinary quality assurance programs that monitor the performance of the practice according to written policies
• Lead a team on creating a Transgender and LGBTQ program
• Lead a team on creating a same day/urgent care program

• EHR LEAD PHYSICIAN Independently studied NextGen® EHR, made agency decisions on which templates to use and how to use them. Assisted the IT team in the system configuration for EHR, including mapping all labs into the NextGen® Compendium. Created all of the workflows for Family Practice, Pediatrics, OB/Women's Health, MA visits, RN walk in visits and Triage visits. Created the training plan, schedule and materials . Conducted all on site training to providers, ma's, and RN's. Provided individual support to providers and their team on their Go-Live week. Developed future workflows for non clinical departments.

• Supervisor to 15 providers and the Clinical Service Manager Supervisor to the Referrals and Medical Records departments Director of the Laboratory Management,in conjunction with the Clinical Service Manager, of the Nursing department, including scheduling, provider assignment, team building and training sessions to improve and develop nursing and communication skills.

• Lead physician in the ACE (Alameda County of Excellence) program.The Alameda County Excellence (ACE) program provides an enhanced health benefits plan to all eligible and enrolled residents of Alameda County with incomes at or below 200 percent of the federal poverty level (FPL).This plan emphasizes chronic disease management through preventive primary care at a designated medical home.The target population is the high-cost, chronically ill.The goal of ACE is to coordinate access and delivery of services, and to focus the design of care to reflect the needs of chronically ill enrollees and those with sufficient risk- factors that may lead to an additional chronic condition.
